Dialysis Liaison Support Specialist Job Description

At Humber River Health, we're committed to delivering exceptional care to our patients. As a Dialysis Liaison Support Specialist, you'll play a vital role in our Nephrology Program, working closely with our team to provide home haemodialysis treatments.

Key Responsibilities:

• Assist patients with haemodialysis treatments in their homes, promoting quality of life and adherence to treatment plans.
• Maintain accurate records and documentation, ensuring seamless communication with our healthcare team.
• Continuously observe patient condition and respond to machine alarms, reporting status changes to the primary registered nurse.
• Assist with supply management and ensure correct equipment use, following established protocols and guidelines.
• Participate in quality assurance and improvement activities, contributing to our commitment to excellence in patient care.

Requirements:

• Personal Support Worker certificate or equivalent
• Minimum 1 year experience in a related field
• Valid driver's license and reliable vehicle
• Current BCLS & CPR Certificates
• Effective communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
