Nursing Care Professional for Geriatric Patients

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Southeast Personal Care Home. The successful candidate will be responsible for providing professional nursing services to our residents, focusing on geriatric care and complex continuing care.
Key Responsibilities- Patient Care: Provide independent nursing services, administer prescribed medical treatments, and collaborate with the healthcare team to ensure comprehensive care.
- Assessment and Planning: Conduct initial and ongoing assessments to identify health needs, develop individualized care plans, and prioritize patient care based on assessments.
- Implementation: Organize work efficiently, perform prescribed interventions, and delegate care appropriately to ensure safe and effective delivery of patient care.
- Professional Responsibility: Provide care in a professional manner consistent with LPN standards and Code of Ethics, report any concerns or issues to the Manager or designates.
