Job Overview

We are seeking an experienced Registered Nurse to join our team as a Clinical Nurse Practitioner in the geriatric care unit.

About the Role

The successful candidate will be responsible for providing high-quality patient care, working closely with the multidisciplinary team to develop and implement individualized care plans. Key responsibilities include assessing patients' physical and emotional needs, administering medications, and collaborating with healthcare professionals to ensure seamless transitions of care.

Requirements
  • Demonstrated expertise in chronic illness management and gerontological nursing practice.
  • Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to prioritize care for multiple patients simultaneously, with attention to detail and commitment to maintaining a safe and respectful environment.
  • Familiarity with electronic health records and other relevant software systems.
