About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Wellness Facilitator Physiotherapist to join our Hants Health and Wellness Team (HHWT). As a key member of our interdisciplinary team, you will play a vital role in providing wellness programming and navigation, chronic disease management, nutrition education, and emotional wellness counselling to our community.

Key Responsibilities:

  • Provide individual and group-based support to patients with chronic diseases, utilizing critical thinking and clinical judgment to develop effective management plans.
  • Collaborate with HHWT staff, primary care providers, and community groups to maintain awareness of existing supports within the community and surrounding areas.
  • Develop and implement community development, population health, and collaborative approaches to support self-management and wellness.
  • Work with patients to identify and address barriers to health and wellness, providing education and support to promote healthy behaviors.
  • Contribute to the development and implementation of programs and services that are responsive to community needs.

Requirements:

  • Bachelor's degree in physiotherapy from an accredited university.
  • Membership with the Nova Scotia College of Physiotherapists.
  • Eligibility for membership with the Canadian Physiotherapy Association.
  • Minimum one year experience in each of the following: working with people with chronic disease in an exercise program or setting, working with community groups/partners building trusting relationships, and individual and group education/facilitation.
  • Understanding of population health, health promotion strategies, and behavior change principles.
  • In-depth knowledge of local health and community-based services.
  • Demonstrated person-centered/family-focused, diversity-focused approaches to care.
  • Well-developed interpersonal, teamwork, oral and written communication skills.
  • Demonstrated flexibility, adaptability, and willingness to change.
  • Demonstrated innovation in program planning, implementation, and evaluation, and quality improvement planning skills.
  • Computer skills (MS Office, email, and internet).
  • Annual BLS AED certification and WHMIS training and certification.
  • RQI training.
  • Valid Nova Scotia driver's license and daily access to a reliable vehicle.
  • Competencies in other languages preferred; French an asset.
