About the Opportunity

The Nova Scotia Health Authority is seeking a highly skilled and dedicated Wellness Facilitator-Physiotherapist to join our Hants Health and Wellness Team (HHWT). As a key member of our interdisciplinary team, you will play a vital role in supporting patients with chronic disease management and wellness through a community development, population health, and collaborative approach.

About You

  • We are looking for a candidate with a Bachelor's degree in physiotherapy from an accredited university and membership with the Nova Scotia College of Physiotherapists.
  • Experience in working with people with chronic disease in an exercise program or setting, as well as community groups/partners building trusting relationships, is essential.
  • You should have a strong understanding of population health, health promotion strategies, and behaviour change principles.
  • Excellent interpersonal, teamwork, oral and written communication skills are required.
  • Computer skills, including MS Office, email, and internet, are necessary.

Responsibilities

  • Support patients with chronic disease management and wellness through individual and group-based settings.
  • Collaborate with HHWT staff, primary care providers, other health care providers, and community groups to maintain awareness of existing supports within the community.
  • Develop and implement community-based programs and services that promote health and wellness.
  • Provide education and facilitation to patients and community groups on health promotion strategies and behaviour change principles.

Requirements

  • Valid Nova Scotia driver's license and daily access to a reliable vehicle.
  • Competencies in other languages, particularly French, are an asset.
