As a Nurse in our Adult Program, you will play a critical role in supporting clients who require complex care and specialized nursing skills to live independently at home.
Key Responsibilities- Assess and monitor clients' needs, developing and implementing personalized care plans to ensure optimal outcomes.
- Evaluate the effectiveness of care plans, making adjustments as needed to ensure the highest level of care.
- Collaborate with clients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide specialized nursing services, including wound care, IV therapy, and injections, as well as attain specialized clinical skills to support complex clients.
- Conduct mental health assessments and provide support to clients, promoting overall well-being and quality of life.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO).
- Membership with the Registered Nurses' Association of Ontario (RNAO) or the Registered Practical Nurses Association of Ontario (WeRPN) is an asset.
- Acute care and medical-surgical nursing experience is an asset.
- Knowledge of community nursing and ability to work independently is an asset.
- Valid First Aid and CPR certification.
- Reliable attendance, punctuality, and availability for the role.
