Geriatric Nursing Specialist - Complex Care Position Overview

Baycrest Hospital has an exciting opportunity for a skilled Geriatric Nursing Specialist to join our team in the Complex Continuing Care Unit on 5 West Hospital.

This is a permanent part-time position that requires the delivery of high-quality, patient-centered care to older adults with complex medical conditions in a non-acute hospital setting.


We are looking for a compassionate and experienced Registered Practical Nurse who is passionate about geriatric care and can provide exceptional care to our patients.

The successful candidate will have a strong commitment to the Complex Continuing Care Program, which focuses on helping patients and their families identify strategies that support quality-of-life goals as well as short-term and long-term care needs.


Key Responsibilities
  • Deliver individualized nursing interventions, including medications and treatments, to meet the personal care, activation, recreation, and spiritual needs of clients.
  • Work collaboratively with the Registered Nurse to develop and revise individualized care plans that address the unique needs and goals of each client.
  • Provide education and support to clients and their families to promote health, well-being, and independence.

Qualifications
  • Current registration in good standing with the College of Nurses of Ontario ('CNO')
  • Basic Cardiac Life Support ('BCLS') certificate, Level C - Automated External Device ('AED')
  • Experience in caring for geriatric patients with complex medical care needs, preferably in a non-acute hospital setting
  • Excellent interpersonal, communication, and problem-solving skills
  • Basic computer skills, including proficiency in Meditech, Microsoft Word, and Excel

This permanent part-time position offers a competitive salary, with an estimated hourly rate of $36.46 - $37.05 and a bi-weekly hours of 30 hours.
