We are seeking a highly skilled and compassionate Clinical Social Worker to join our In-Patient Psychiatry team at Fraser Health. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive social work services and support.
Key Responsibilities- Conduct comprehensive social work assessments and develop individualized care plans to address the unique needs of patients and their families.
- Provide counseling and therapy services to patients and families, including crisis intervention and support.
- Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between hospital and community-based services.
- Develop and deliver educational programs and workshops to promote mental health awareness and support.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in social work.
- Master's degree in Social Work (MSW) from an accredited program.
- Current registration with the British Columbia College of Social Workers.
- Minimum one year of experience in a mental health treatment setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong knowledge of mental health issues, including substance misuse and addiction.
