About the Role:- Conduct comprehensive social work and psychosocial assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
- Develop and coordinate care plans, community services, and discharge planning to ensure optimal continuity of care.
- Provide education, support, and consultation to patients, families, and healthcare professionals on issues related to mental health, child protection, and community living.
- Maintain patient records, provide therapeutic clinical counselling services, and participate in program evaluation and planning.
- Develop, coordinate, and deliver in-services, presentations, and training related to mental health.
- Conduct comprehensive social work and psychosocial assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
- Develop and coordinate care plans, community services, and discharge planning to ensure optimal continuity of care.
- Communicate and collaborate with mental health and related community service agencies and family physicians.
- Act as a resource to patients and their families, providing information and facilitating access to community resources.
- Educate and provide consultation and support to patients, families, and healthcare professionals on issues related to mental health.
- Maintain patient records and provide therapeutic clinical counselling services.
- Participate in program evaluation and planning, and develop, coordinate, and deliver in-services, presentations, and training related to mental health.
- Master's Degree in Social Work from an accredited school of Social Work.
- One year's recent, related experience in a mental health treatment setting.
- Current full registration with the BC College of Social Workers.
- Current Class 5 Driver's License and access to a personal vehicle for business-related purposes.
- Comprehensive knowledge and understanding of emotional issues related to mental illness and addictions.
- Ability to make effective referrals and collaborate with community health and social agency personnel.
- Comprehensive counselling skills, including crisis counselling, motivational interviewing, and group work.
- Demonstrated ability to organize and prioritize workload in a complex environment.
