### Job Summary

We are seeking a skilled Social Worker to join our team at Interior Health Authority. As a Social Worker, you will play a vital role in providing comprehensive assessment, care planning, and treatment coordination for designated clients and their families.

### Key Responsibilities

* Provide direct psychosocial care, counseling, system navigation, and advocacy to clients and their families
* Collaborate with healthcare providers and the multidisciplinary team to develop and implement care plans
* Facilitate client self-management and provide education on relevant specialized complex care resources
* Develop and maintain social work services to improve the health and social well-being of the identified population

### Scheduling Information

Our department provides Social Work services 7 days a week with varying hours of operation. Shift times range from 08:00 to 16:00, 09:00 to 21:00, and 15:00 to 23:00 hours. This is a Casual opportunity, which means there are no guaranteed hours, and work requirements can vary from hours per week.
