Care Coordinator Role
At pt Health, we are seeking a highly organized and customer-focused Care Coordinator to join our team in Hamilton, ON. As a Care Coordinator, you will play a vital role in fostering strong relationships between our clinic and its customers, including patients, staff, funders, and referral sources.
Key Responsibilities
- Provide exceptional customer service to patients and visitors, answering phone calls, and responding to inquiries in a timely and professional manner.
- Coordinate patient intake, data entry, and scheduling of appointments, ensuring accurate and detailed information is maintained in patient records.
- Assist patients with completing paperwork and ensure accurate information is collected to avoid errors.
- Collect payments at the time of service and track coverage for all funding streams to ensure approval.
- Complete data entry and billing, as well as coordinate courier packages and process incoming and outgoing email, mail, and faxes.
- Provide administrative support to the Care Coordinator Manager and Clinic Director.
Qualifications & Core Competencies
- High School Diploma, G.E.D., or equivalent.
- Excellent customer service skills and telephone etiquette.
- High degree of organizational skills and ability to prioritize and multi-task.
- Excellent communication (verbal/written) and interpersonal skills.
- Strong computer and data entry skills.
- Flexible and adaptable to change.
- Experience in a Physiotherapy clinic an asset.
