Job Title: Wound Care Nurse

St. Joseph's Home Care is seeking a skilled Wound Care Nurse to join our team. As a Registered Practical Nurse (RPN), you will play a vital role in providing high-quality care to our clients with predictable outcomes.

Responsibilities:
  • Conduct comprehensive nursing assessments to identify client needs and develop individualized care plans.
  • Provide wound care management, including dressing changes, wound cleaning, and debridement.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Participate in continuing professional development opportunities to maintain current competence and advance wound care skills.
  • Act as an advocate for clients and caregivers, interpreting information, supporting decisions, and facilitating communication.
Qualifications:
  • Current RPN registration with the College of Nurses of Ontario.
  • Practical Nursing Diploma from an accredited college program with evidence of IV medication administration.
  • Wound care certificate from an accredited institution.
  • Basic Cardiac Life Support (BCLS) certification.
  • Ability to function to the full scope of practice as per the College of Nurses of Ontario.
