Job Summary

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at Providence Health Care. As a key member of our interdisciplinary team, you will provide therapeutic clinical counseling services, conduct comprehensive biopsychosocial assessments, and maintain patient/resident records.

Key Responsibilities
  • Provide therapeutic clinical counseling services to patients/residents, families, and groups in designated program areas.
  • Conduct comprehensive social work and psycho-social assessments in designated program areas.
  • Maintain patient/resident records, including documenting patient/resident assessments, charting patient/resident information, and preparing progress notes and reports.
  • Contribute to the discharge planning process as a member of the interdisciplinary care team.
  • Provide consultative and direct services to patients/residents, families, staff, and community health professionals regarding community resources and access to these resources.
  • Participate in clinical care team conferences as a member of the interdisciplinary care team for patient/resident reviews, admissions, and discharges.
  • Support patients/families/teams in understanding legislative processes and criteria related to healthcare consent, medical assistance in dying, and adult guardianship.
  • Provide ongoing program evaluation and develop, coordinate, and deliver in-services, presentations, and/or training related to designated programs for staff.
  • Participate as a member of local committees and facilitate/participate in group meetings.
  • Maintain broad knowledge of developments in the Social Work profession and develop research protocols and conduct approved social work interdisciplinary research and special projects.
  • Provide teaching and supervision to social work students as assigned.
Requirements
  • Masters in Social Work from an accredited university.
  • One year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience.
  • Current full registration with the British Columbia College of Social Workers.
