Nurse Care Coordinator for Medical Integrated Program

SalaryWe offer a competitive salary range of $80,000 - $110,000 per year, depending on experience.
Job DescriptionAs a key member of our team, you will play a vital role in coordinating patient care within our Medical Integrated Program. This program encompasses three floors (B6, B7, and B8) with a varying patient population requiring acute care services. Our hospitalist-led model of care brings together RNs, RPNs, and PSWs to provide comprehensive care, and as a Nurse Care Coordinator, you will have the opportunity to cross-train between all three floors, enhancing your skills and contributing to the growth and development of your colleagues.
The successful candidate will have excellent interpersonal and communication skills, with the ability to work independently and collaboratively within an interprofessional team. A self-directed individual with a passion for continuing education and personal development in nursing, you will bring creative problem-solving skills, a patient-friendly approach, and a demonstrated commitment to exceptional customer service.
Required Skills and Qualifications- Graduate of an accredited college nursing program
- Registered and in good standing with the College of Nurses of Ontario
- Current Basic Cardiac Life Support (BCLS) certification
- Two (2) years of experience in a medical/surgical setting
