VON Canada (Ontario) is seeking a highly skilled and organized individual to fill the role of Care Coordinator in our Client Service Team.

The Role:

We are looking for a dedicated and customer-focused professional to coordinate client visits, manage scheduling, and ensure quality care delivery. The successful candidate will possess excellent problem-solving skills, be tech-savvy, and have experience working in a fast-paced environment.

Key Responsibilities:

  • Assign client visits to nurses and home support workers based on local geography, client needs, and continuity of care guidelines.
  • Process client referrals, respond to phone inquiries, and perform advanced scheduling tasks.
  • Work closely with team members and management to ensure adequate staffing levels and quality client care delivery.
  • Support clients, nurses, and home support workers to ensure seamless care delivery.
  • Leverage technology to optimize scheduling, communication, and data management.

Requirements:

  • Possess an Office Administration Certificate, Medical Terminology Certificate, or equivalent experience.
  • Be proficient in MS Office programs, Outlook, faxing software, and database management.
  • Demonstrate experience in logistics or scheduling environments and proficiency in scheduling software like Procura.
  • Have a strong background in customer service, preferably in a contact center environment.
