Senior Community Care Liaison

About This Role
">We are seeking a highly skilled Senior Community Care Liaison to join our team at Fraser Health. In this role, you will be responsible for providing exceptional care and support to our clients in the community.
Key Responsibilities
- Assess and monitor client needs on an ongoing basis, identifying areas for improvement and developing targeted plans to address these needs
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each client
- Provide education and support to clients and their families, empowering them to take an active role in their healthcare
- Act as a liaison between acute care facilities and community care services, facilitating smooth transitions and ensuring continuity of care
- Develop and maintain effective relationships with community resources and service providers, ensuring seamless access to necessary support services
Requirements
- Graduation from an approved program of Practical Nursing, plus one year recent related acute care, residential and/or community-based clinical experience, or an equivalent combination of education, training and experience
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required
