Job Summary

We are seeking a compassionate and organized Senior Care Coordinator to join our team at Home Instead. As a key member of our office staff, you will play a vital role in coordinating services for our clients, ensuring their safety and well-being.

About the Role

The Senior Care Coordinator will be responsible for assessing client needs, developing personalized care plans, and coordinating with healthcare professionals to provide comprehensive support. This includes conducting initial evaluations and ongoing assessments, creating tailored service plans, and communicating effectively with clients, family members, and caregivers.

Key Responsibilities:
  • Conduct thorough client assessments to identify needs and preferences
  • Develop and implement comprehensive care plans that meet individual client goals
  • Coordinate with healthcare professionals, such as physicians, therapists, and nurses, to ensure a multidisciplinary approach to client care
  • Communicate effectively with clients, family members, and caregivers to ensure seamless service delivery
  • Monitor client progress, update care plans, and address any concerns or issues that arise
Requirements

To succeed in this role, you should have excellent communication and organizational skills, as well as a genuine passion for providing exceptional care to seniors. A Class 5 driving license and access to a reliable vehicle are also essential, as some travel may be required to visit clients in Burnaby and South Vancouver.
