Job Description

Pacific Senior Solutions Inc, an independently owned and operated Home Instead franchise, is seeking a compassionate Health Care Professional to fill the role of Senior Care Coordinator.

This exciting opportunity involves working closely with our office team to enhance the lives of aging adults in our community. As a key member of our team, you will be responsible for coordinating client care services, conducting thorough assessments, and developing comprehensive care plans that cater to each individual's unique needs.

The ideal candidate will possess excellent communication skills, the ability to work effectively with healthcare professionals, and a commitment to providing exceptional care to our clients.

Key Responsibilities:
  • Conduct initial evaluations and ongoing assessments to identify client needs and preferences.
  • Develop personalized care plans that outline specific goals and support services required.
  • Maintain open communication with stakeholders, including family members, healthcare providers, and referral sources.
  • Collaborate with healthcare professionals to ensure a multidisciplinary approach to client care.
  • Identify potential issues and implement solutions to maintain client well-being and satisfaction.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as extended health care benefits and on-site parking. This full-time position requires occasional travel within Burnaby and South Vancouver, so a valid Class 5 driving license and reliable transportation are essential.
