Job Summary

We are seeking a highly skilled Pharmacist to join our team as a Pharmacy Excellence Coordinator. In this role, you will be responsible for providing exceptional patient-centered care and exceeding patients' expectations.

Responsibilities

  • Provide clinical services focused on continuing care contractual obligations, including disease state management, medication reviews, and reporting.
  • Initiate care plans, follow up, and complete proper documentation.
  • Participate on facility committees and complete biannual medication reviews with Nurses, Physicians, and Nurse Practitioners.
  • Ensure prescription safety and prioritize technical functions to Pharmacy Registered Technicians and Assistants as required.
  • Provide quarterly reports to facility administration and review all medication errors and complete audits.
  • Maintain current knowledge of pharmacy practice, regulatory requirements, and industry trends/developments.
  • Adhere to all regulatory requirements, Company policies, processes, and procedures.

Requirements

  • A Bachelor of Science in Pharmacy from a recognized institution.
  • A Licensed Pharmacist, in good standing, in the province of responsibility.
  • Kroll software experience is preferred.

Salary Estimate

The estimated annual salary for this role is approximately $85,000-$110,000, based on industry standards and the location of the position.
