Professional Care Coordinator

Job Summary
We are seeking a Professional Care Coordinator to join our team at Fraser Health. As a key member of our interdisciplinary team, you will be responsible for providing psychosocial assessments, identifying patient and family issues, and formulating treatment plans that meet the unique needs of each client.
About the Role
In this rewarding role, you will provide clinical and supportive counselling services to patients and families, working collaboratively with healthcare professionals to address social, emotional, and cultural concerns. You will also provide crisis intervention, support services, and education to individuals, couples, families, or groups.
Key Responsibilities
- Conduct thorough psychosocial assessments to identify patient and family needs
- Develop and implement individualized treatment plans that address social, emotional, and cultural concerns
- Provide clinical and supportive counselling services to patients and families
- Crisis intervention and support services
- Education and support for individuals, couples, families, or groups
Qualifications
To be successful in this role, you must have a Master's degree in Social Work from an approved school of Social Work, plus two years' recent, related clinical experience. You must also be registered with the BC College of Social Workers.
Salary and Benefits
The salary for this position is $ $83,413 per year, plus comprehensive benefits and opportunities for professional growth and development.
