Nurse Care Coordinator

We are seeking a detail-oriented and compassionate Nurse Care Coordinator to join our team at Fraser Health.
About the Role:
- Collaborate with our multidisciplinary team to provide high-quality patient care.
- Assess, document, and report patient health status to ensure effective care planning.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Communicate effectively with patients, families, and healthcare professionals.
Responsibilities:
- Administer medications and perform various nursing functions as needed.
- Conduct physical, mental, emotional, and social assessments to inform care plans.
- Develop and modify health care plans with input from patients, families, and healthcare teams.
- Evaluate patient response to care plans and make adjustments as necessary.
Requirements:
- Current registration with BCCNM as an Employed Student Registrant.
- Enrollment in a recognized nursing education program.
- Certification in Basic Cardiac Life Support.
- Successful completion of a medical/surgical or mental health/substance use clinical practicum.
Salary Range: $50,000 - $65,000 per annum based on experience.
