We are seeking a highly skilled and compassionate Case Manager to join our Assertive Community Treatment Team (ACTT) in the South East region. As a Case Manager, you will work closely with clients who are experiencing significant difficulties in day-to-day and role functioning due to serious and persistent mental illness and/or addictions.
Key Responsibilities:- Collaborate with clients to identify strengths and needs, and develop long-term goals, short-term objectives, and concrete action steps to achieve these objectives.
- Provide individualized and flexible services to engage, support, and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential.
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety.
- Participate in individual and systemic advocacy initiatives to improve resource access and availability.
- A degree or diploma in the health or social services.
- One or more years of experience in a community-based organization working with adults with serious mental illness in the community.
- Full registration and in good standing with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.
- Experience working with individuals with Addictions is an asset.
- Second language (Cantonese, Mandarin, Farsi or Tamil, Italian, Korean, Punjabi, Russian, Urdu) reflecting the local community is an asset.
