About the Role

We are seeking a skilled Case Manager to join our Assertive Community Treatment Team (ACTT) in South East York Region. As a Case Manager, you will work with clients who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions.

Key Responsibilities

  • Collaborate with clients to identify strengths and needs, and develop long-term goals, short-term objectives, and concrete action steps to achieve these objectives.
  • Assist clients in acquiring and using skills, and connecting with formal and informal supports, peer support, professional services, and resources to promote their recovery, improve their quality of life, and reduce the likelihood of relapse.
  • Provide individualized and flexible services to engage, support, and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential.

Requirements

  • A degree or diploma in the health or social services, with preference given to a degree in a related discipline.
  • One or more years of experience in a community-based organization working with adults with serious mental illness in the community.
  • Full registration and in good standing with the OCSWSSW or a regulatory body under the Regulated Health Professions Act.
