About the Role

The Nurse Case Manager will play a key role in supporting patients with reimbursement support to physicians and medical case management services. The successful candidate will assist physicians with the completion of program-specific paperwork and follow up on submissions to ensure patients have access to their prescribed treatments in a timely manner.

This is a remote contract position for up to 16 months, open to candidates residing in any Canadian province or territory. The ideal candidate will be available to work from 9:00am to 5:00pm Eastern Standard Time (EST).

A Day in the Life

  • Assist in completing patient enrolment, including gathering pertinent information to complete necessary forms in a concise and organized manner.
  • Liaise and advocate with third-party providers and provincial governments to determine coverage options.
  • Review patient charts and ensure data integrity.
  • Ensure patients are scheduled for necessary diagnostic tests and notify physicians of expired test results.
  • Complete Welcome Call activities with patients according to defined scripts.
  • Review Post Injection Reports to confirm next appointment and data integrity.
  • Track missing Post Injection Reports.
  • Facilitate communication with physicians, Patient Support Programs, and other healthcare professionals.
  • Follow up and advocate until a decision is received, securing coverage for patients.
  • Work closely with manufacturer local sales representatives to support physician and staff needs.
  • Offer and/or provide educational training to patients and physicians on product administration.
  • Act as a central resource liaison for program stakeholders.
  • Coordinate and schedule patients at appropriate injection locations.
  • Provide updates to stakeholders based on observations.
  • Ensure patient records are maintained and data-accurate to meet program reporting requirements.
  • Identify and report Adverse Events to Health Canada, Med Info, and enter into source systems.
  • Participate in quality activities and continuous improvement initiatives.
  • Maintain confidentiality of client and corporate information.
  • Complete all relevant reports, including time sheets, expenses, and mileage.

Job Qualification

The ideal candidate will possess:

  • A graduate degree in nursing or a related field.
  • Current registration with a provincial licensing body in Canada.
  • Minimum 2 years' experience in Patient Support Programs.
  • Oncology experience is considered a valuable asset.
  • Excellent customer service skills and fluency in verbal and written communication and interpersonal skills for both English and French.
  • Strong computer and technical skills, including Excel, Word, Outlook, and web portals.
  • Strong critical thinking and problem-solving skills.
  • Ability to work in a team environment and resolve conflicts professionally.
  • Strong sense of organization and attention to detail.
  • Self-regulation of time management and ability to multi-task and adhere to deadlines.
