Role OverviewThe Health Care Social Worker reports to the Program Manager and provides interventions to clients and their families, enabling them to achieve their optimal level of functioning and health.
These interventions include psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning, and case management. The role involves advocacy, education, program planning, consultation, and collaboration on behalf of individuals or groups within the institution and community.
Key Responsibilities- Provide psychosocial assessments and develop interventions to address client needs
- Work collaboratively with multidisciplinary teams to provide comprehensive care
- Advocate for clients and families, addressing social determinants of health
- Develop and implement education programs to promote healthy behaviors and disease prevention
- Collaborate with community agencies and organizations to provide support services
- Master's Degree in Social Work from a recognized university
- Registered or eligible for registration with the NSCSW
- Two (2) years of clinical experience as a clinical Social Worker preferred
- Strong counselling and communication skills
- Capacity to achieve a high degree of autonomous practice
- Ability to work within a multidisciplinary team context
- Experience providing care in the emergency department an asset.
- Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving, and continuum of care
- Competencies in other languages an asset, French preferred
- $37.31 - $49.05 Hourly salary range
- Social Worker (Masters) without 2 years of experience (HTH130) - $33.27 - $36.02 Hourly salary range
