Patient Flow Navigator

Bayshore HealthCare is seeking a highly skilled Patient Flow Navigator to join our team. This exciting opportunity allows you to make a meaningful impact on the lives of patients and their families.
Job SummaryThe Patient Flow Navigator plays a critical role in facilitating patient flow within our healthcare system. As a key member of our team, you will be responsible for assessing patients against eligibility criteria, coordinating care transitions, and ensuring seamless patient flow.
Responsibilities- Patient Assessment: Conduct thorough assessments of patients to determine their suitability for transitional care.
- Care Coordination: Collaborate with healthcare teams to develop and implement individualized care plans that meet patients' unique needs.
- Flow Optimization: Identify and address barriers to patient flow, implementing strategies to improve efficiency and reduce wait times.
- Communication: Maintain effective communication with patients, families, and healthcare providers to ensure timely and accurate information exchange.
To be successful in this role, you will require:
- Education: A Bachelor's degree in a health discipline (Nursing, Physiotherapy, Occupational Therapy) or a College diploma in Nursing.
- Registration: Current registration and membership with a regulated college.
- Experience: 3-5 years of clinical experience in a healthcare organization.
- Skills: Excellent organizational, interpersonal, and problem-solving skills; demonstrated agility in decision-making and critical thinking.
We offer a competitive salary range of $80,000 - $110,000 per annum, commensurate with experience.
