Job Summary

We are seeking a skilled Registered Practical Nurse/Licensed Practical Nurse to join our team as a Healthcare Professional - Client Care Specialist. This role will involve establishing, monitoring, and delivering nursing care to clients in their homes, as per the nursing care plan and in accordance with professional practice standards.

About the Role

The successful candidate will be responsible for providing client care by utilizing observation, assessment, nursing diagnosis, counseling, and health teaching skills. They will also be responsible for performing routine nursing duties in accordance with company policies and procedures, as well as maintaining confidentiality of client and corporate information.

Key Responsibilities

  • Establish, monitor, and deliver nursing care to clients in their homes
  • Provide care according to the established care plan, ensuring the emotional, spiritual, physical comfort, and safety of clients are met to the greatest possible extent
  • Perform routine nursing duties in accordance with company policies and procedures
  • Maintain confidentiality of client and corporate information
  • Participate in ongoing internal and/or external continuing education activities

Required Skills and Qualifications

  • Graduation from an accredited Licensed/Registered Practical Nursing Program
  • Current registration with the provincial licensing body
  • Previous experience in the acute care sector or long-term care
  • Demonstrated clinical competencies and positive interpersonal skills
  • Ability to communicate effectively both verbally and written
  • Critical thinking and analytical skills, decision-making skills, and ability to prioritize
