We are seeking a dynamic and passionate Registered Nurse to join our Transitions team in Canada. As a key member of our interdisciplinary care team, you will be responsible for providing comprehensive care to clients in their homes and communities.

Job Description

As a Dynamic Community Care Specialist, you will be responsible for:

  • Providing primary contact for clients when designated as the primary care provider/clinician for the client's interdisciplinary team
  • Facilitating regular interdisciplinary team communication (e.g. huddle, rounds, status updates) when designated as the primary care provider/clinician
  • Leading discharge and transition care planning discussions during regular integrated team meetings with their front-line staff when designated as the primary care provider/clinician
  • Supporting clients through care transitions by collaborating and connecting them with community support services in their local community
Required Skills and Qualifications

To be successful in this role, you will need:

  • A minimum of two years of nursing experience
  • Experience in one of the following areas: home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
  • Diverse nursing experiences with different client groups will be an asset
  • Community experience is an asset
