As a Registered Nurse Navigator in the Combined Surgical Unit's (CSU) Nursing/Multi-disciplinary team, you will practice in accordance with relevant legislation/standards, meeting the legal and ethical requirements of the profession.

About the Role

As a member of our team, you will support patients as they manage their health and navigate the healthcare system in various settings, including community, other healthcare organizations, and home appointments. Your core responsibilities include collaboration in post-operative assessment and estimated discharge determination for surgical populations. You will also work closely with an inter-disciplinary team to provide person-centered support for individuals recovering from surgery and families living with chronic disease.

Duties and Responsibilities
  • Collaborate with healthcare professionals to develop and implement effective care plans for patients undergoing surgery.
  • Provide education and support to patients and their families on managing post-operative care and navigating the healthcare system.
  • Conduct assessments and identify areas for improvement in patient care, contributing to quality improvement initiatives.
  • Work collaboratively with the interdisciplinary team to ensure seamless transitions of care and minimize readmissions.

Compensation and Benefits

We offer a competitive salary of $90,000 - $110,000 per year, depending on experience.
