Patient Care Coordinator

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at beBee Professionals. As a key member of our healthcare team, you will be responsible for providing high-quality patient care, conducting thorough nursing assessments, and developing individualized care plans.
Key Responsibilities:- Conduct comprehensive nursing assessments to identify patient needs and develop individualized care plans.
- Collaborate with the healthcare team to implement patient care plans and ensure seamless care delivery.
- Administer medications and monitor patients' responses to treatment, documenting all interactions and outcomes.
- Educate patients and families about health management and wellness, promoting self-care and independence.
- Maintain accurate and up-to-date patient records, adhering to regulatory requirements and professional standards.
- Valid LPN license in Canada.
- Completion of an accredited Practical Nursing program.
- Experience in a clinical or healthcare setting is an asset.
- Strong communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
