Community Visit Nursing; Registered Practical Nurses (RPN/LPN)

About the Role

We are seeking passionate and caring Registered Practical Nurses (RPN/LPN) to join our team at Bayshore HealthCare. As a Community Visit Nurse, you will use your clinical skills to deliver high-quality home health care to our clients, ensuring their comfort and safety in their own homes.

Key Responsibilities

  • Provide exceptional care to clients in their homes, utilizing your clinical skills and knowledge to meet their unique needs.
  • Conduct thorough assessments and develop personalized care plans to ensure optimal outcomes.
  • Collaborate with our interdisciplinary team to coordinate care and services.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care transitions.
  • Participate in ongoing education and training to enhance your skills and knowledge.

Requirements

  • A current nursing license in good standing with the College of Nurses of Ontario (CNO).
  • Graduation from an accredited School of Nursing.
  • At least two years of nursing experience, with community experience an asset.
  • Proof of a current Negative TB test.
  • A current/clear vulnerable sector check.
  • Excellent assessment, documentation, verbal, and communication skills.
  • A valid driver's license (G Class) and access to a reliable vehicle.
