Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Connecting Care. As a key member of our community, you will be responsible for delivering high-quality person-directed care to our residents.
Key Responsibilities- Resident Care: Ensure top-notch care is provided to our residents, with a focus on time management and prioritization.
- Team Collaboration: Work collaboratively with our team to achieve person-directed services and maintain a positive, professional work environment.
- Medication Assistance: Provide medication assistance to residents, adhering to our Well-Being Strategies and accepted practice standards.
- Clinical Mentorship: Mentor and delegate nursing tasks to Care Partners, promoting effective advocacy for residents and their well-being.
- Quality Improvement: Identify and report health, safety, or maintenance issues, and participate in ongoing quality improvement initiatives.
- Communication: Liaise with doctors, pharmacists, homecare nurses, managers, families, and other stakeholders to ensure seamless care delivery.
- Education: Graduate of a recognized LPN program.
- Licensure: Registered and in good standing with the CLPNA.
- Certifications: Valid certification in First Aid and CPR.
- Criminal Record Check: Obtain a clear criminal record check with vulnerable sector check prior to employment.
