Job Title: Home Support Supervisor

At Spectrum Health Care, we are seeking a highly skilled and experienced Home Support Supervisor to join our team. As a key member of our leadership team, you will be responsible for leading a team of Personal Support Workers to provide high-quality care to patients in their homes.

Key Responsibilities:
  • Lead a team of Personal Support Workers to provide qualified personal care to patients in a home setting
  • Travel to patients' homes in community and facility settings to provide support and guidance
  • Collaborate with multidisciplinary teams in both community and facility settings to ensure seamless care delivery
  • Assess patient needs and develop individualized care plans to achieve optimal outcomes
  • Manage staff performance, conduct performance appraisals, and implement progressive disciplinary action as needed
  • Monitor patient and employee feedback, and maintain awareness of key performance indicators such as caseload volume, staff turnover, and occurrence rates
  • Address patient concerns and complaints in a timely and professional manner
  • Promote Spectrum Health Care through participation in local healthcare committees and community events
  • Participate in ongoing internal and external continuing education to enhance personal and professional development
Qualifications:
  • Current registration with the Colleges of Nurses of Ontario in good standing
  • Minimum three years' experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting
  • Exceptional interpersonal skills and ability to handle difficult situations in a professional and objective manner
  • Excellent written and verbal communication skills, with the ability to write reports and communicate effectively
  • Valid driver's license and access to a reliable vehicle
  • Basic proficiency in Microsoft Word, Excel, and ability to learn proprietary software
