Job Title: Home Support Supervisor

At Spectrum Health Care, we are seeking a highly skilled and experienced Home Support Supervisor to lead our team of Personal Support Workers in providing exceptional care to patients in their homes.

Key Responsibilities:
  • Lead a team of Personal Support Workers to deliver high-quality patient care in a home setting
  • Travel to patients' homes in community and facility settings to provide support and guidance
  • Collaborate with multidisciplinary teams to ensure seamless care delivery
  • Assess patient needs, develop care plans, and monitor progress towards outcomes
  • Manage staff performance, conduct appraisals, and implement disciplinary actions as needed
  • Collect and analyze patient and staff feedback to identify areas for improvement
  • Address patient concerns and complaints in a timely and professional manner
  • Promote Spectrum Health Care through community events and partnerships
  • Participate in ongoing education and professional development to enhance skills and knowledge
Qualifications:
  • Current registration with the Colleges of Nurses of Ontario in good standing (RN or RPN)
  • Minimum three years' experience in leadership roles, preferably in a community setting
  • Exceptional interpersonal and communication skills
  • Ability to write reports and communicate effectively in writing and verbally
  • Excellent English language skills (additional languages considered an asset)
  • Solid knowledge of community nursing principles and practices
  • Valid driver's license and access to a reliable vehicle
  • Basic proficiency in Microsoft Office and ability to learn proprietary software
