Job Description

We are seeking a highly organized and detail-oriented Team Assistant to provide administrative support for our assigned team in their daily activities. The successful candidate will be responsible for:



  • Providing administrative support services to Care Coordinators, including processing new referrals, orders, and managing confidential patient records;
  • Entering, updating, and maintaining patient data in the electronic database with high accuracy and efficiency;
  • Answering telephone inquiries from patients, families, and service providers, and referring callers as appropriate;
  • Providing back-up support to other positions, as required.


Required Skills and Qualifications

To succeed in this role, you will need:



  • A college diploma in health or social services, business/office administration, or related field;
  • 2+ years of related office experience, with strong keyboarding/data-entry skills and proficiency in database software, MS Word, and Excel;
  • Excellent organizational skills, ability to work with minimal supervision, and advanced multi-tasking skills;
  • Exceptional interpersonal skills, ability to work as part of a team, and interact tactfully and sensitively with patients from diverse backgrounds;
  • Flexibility to work a schedule that includes days, evenings, and weekends to meet organizational needs.
