Key Responsibilities- Collaborate with patients and their families to assess care needs and develop individual care plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
- Provide access and referrals to community services, ensuring patients' values and preferences are respected
- Membership in good standing with the applicable regulatory body
- University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, or related field
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
