Job Title: Rehabilitation Counsellor

Join our team at the Aboriginal Health and Wellness Centre as a Rehabilitation Counsellor, providing long-term case management, systems navigation, and counselling services to individuals in need of care.

Job Summary

We are seeking a skilled and compassionate Rehabilitation Counsellor to join our team at the Indigenous-led RAAM Clinic. As a key member of our inter-professional team, you will provide intake and assessment, counselling, referrals, educational sessions, and continuing care services for individuals struggling with addiction and mental health issues.

Key Responsibilities
  • Provide culturally sensitive and person-centered care to Indigenous and non-Indigenous community members
  • Develop and implement treatment plans in collaboration with the RAAM team and other healthcare professionals
  • Facilitate problem-solving techniques with clients and support a Collaborative Care Model
  • Document client information in a clear and concise manner, adhering to policies and legal guidelines
  • Collaborate with withdrawal management services, addiction treatment agencies, and other external agencies to provide comprehensive care
Requirements
  • Completion of a recognized degree or diploma in Social Services or a related health discipline
  • Minimum six months of experience working with individuals and families managing addiction
  • Basic familiarity with addiction and mental illness presentations, as well as knowledge of relevant legislation and policies
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills
