Job Title: LPN/RN Home Care

Edgewood Health Network Inc is seeking a skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team in Home Care. As a member of our interdisciplinary team, you will provide medical care, recovery therapies, and everyday care and support to clients in Detox and addictions residential treatment.

Key Responsibilities:
  • Provide nursing care to clients, administer medications in accordance with organizational and pharmaceutical policies and procedures.
  • Support the Nursing Team and Physician as directed, including clinical nursing assessments during the patient intake and discharge process.
  • Gather medical/mental health/substance use history, collect urine for substance screening, and administer breathalyzer tests.
  • Provide nursing care and interventions, vital sign monitoring, and medication administration for patients during medically supervised detox process.
  • Monitor acute or chronic conditions, administer scheduled and PRN medications, and perform First Aid.
  • Document nursing assessments, nursing care, behaviors, and related progress in KIPU electronic medical record system.
  • Communicate with physician regarding acute medical concerns, medication orders, detox protocols, patient progress, medical services required, and discharge prescriptions.
  • Collaborate with Physician, Clinical Counselors, Aftercare and Admissions Coordinators, Support Staff, and the Administrative Team to maintain continuity of care.
Requirements:
  • Valid and Current LPN License with the College of Nurses of British Columbia.
  • Trauma-informed training/experience an asset.
  • Ability to read and write and communicate verbally and in writing.
  • Valid First Aid/CPR Level C.
  • Valid Class 5 Drivers License in good standing.
