RhynoCare is currently in search of a dedicated and passionate individual to fulfill the role of Nurse Practitioner (NP) within a Long-term Care (LTC) environment.
Key Responsibilities:
- Provide comprehensive primary health care to residents in LTC, addressing both acute and chronic medical conditions, promoting health, and preventing diseases/injuries.
- Offer on-call support as required by operational needs.
- Conduct thorough clinical evaluations utilizing sound judgment and diagnostic skills, adhering to the full scope of practice as outlined by the regulatory body.
- Lead clinical initiatives among LTC team members, focusing on resident care, including admissions, discharges, palliative care, and management of complex health needs.
- Utilize various assessment tools to evaluate residents' health based on individual requirements.
- Compile detailed health histories, considering physical, psychosocial, emotional, cultural, and ethnic aspects.
- Perform physical examinations, identify and interpret findings, and order necessary diagnostic tests while applying evidence-based clinical reasoning.
- Engage with residents and their families to create personalized care plans, synthesizing health information to establish diagnoses and identify health risks.
- Communicate health assessment results and diagnoses to residents, applying knowledge of relevant medical sciences.
- Select appropriate interventions from a variety of treatment options to support residents' health and well-being.
- Ensure safe resident care by addressing potential risks and adverse events proactively.
- Coordinate with discharge planning services to facilitate smooth transitions for hospitalized residents returning to LTC.
- Collaborate with community health services to enhance continuity of care and participate in educational initiatives for the care team.
- Actively engage in quality assurance programs to maintain professional competence.
- Allocate 80% of time to direct resident care and 20% to research and educational activities aimed at improving care standards.
- Incorporate an understanding of diversity and cultural safety in assessments and therapeutic management.
- Exhibit leadership capabilities within an inter-professional team, fostering effective working relationships and providing mentorship.
- Build and maintain partnerships with diverse stakeholders and work collaboratively within multidisciplinary teams.
- Demonstrate strong verbal and written communication skills.
- Exhibit flexibility in work hours, including potential evenings or weekends, and participate in after-hours coverage.
- Possess knowledge of best practice guidelines and relevant regulations.
Essential Qualifications:
- Registered with the College of Nurses of Ontario in the Extended Class.
- Bachelor's degree in Nursing (BScN); a Master's degree in Nursing (MSN) with a Nurse Practitioner specialization is preferred.
- Membership with the Nurse Practitioners' Association of Ontario (NPAO) is an asset.
- Professional liability insurance is required.
- National certification in gerontology is advantageous.
- Ability to manage crises effectively and professionally.
- Strong written communication skills suitable for clinical documentation.
- Effective communication with clients, employing active listening to understand their needs.
- Basic CPR certification is mandatory.
- Current Vulnerable Sector Check (VSC) is required.
- Compliance with immunization mandates as specified by health authorities.
