Job Summary

Southbridge Care Homes is seeking a skilled Registered Practical Nurse to join our team at West Park Health Centre, a 69-bed long-term care facility in Ontario. As a Registered Practical Nurse, you will play a vital role in enhancing the effective management and quality of nursing practice, ensuring the highest standard of care for our residents.

Key Responsibilities

  • Provide resident care in accordance with established nursing standards, principles, and administrative policies.
  • Assess residents, develop, implement, and evaluate resident care plans, monitoring their health and noting changes in condition.
  • Organize and direct functions and activities related to the provision of care, including restorative care and palliative care as required.
  • Plan, direct, supervise, and evaluate the work of personal care staff assigned to the unit, orienting and training new personnel as needed.
  • Ensure safe and accurate medication administration to residents.

Qualifications

  • Current certificate of competence from the College of Nurses of Ontario.
  • Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
  • Comprehensive knowledge of nursing and healthcare practices, including Infection Control Practices and IPAC certification.
  • Experience interacting effectively with persons who have cognitive impairments, using approaches like G.P.A and P.I.E.C.E.S.
  • Previous experience working in Long Term Care in Ontario, with knowledge of standards of care and the ability to uphold them.
  • Effective human relations skills and a demonstrated commitment to working with seniors.
  • Effective communication skills and the ability to work independently and collaboratively.
  • Strong leadership skills and participation in workflow process changes.
  • Familiarity with P.C.C. and e.M.A.R.
  • Current First Aid and C.P.R. certification preferred.
