Job Title: Community and Social Services Worker

We are seeking a dedicated and compassionate Community and Social Services Worker to join our team at Improving Lives In-Home Care. As a key member of our team, you will play a vital role in providing support and care to individuals in their homes, promoting independence and enhancing their overall quality of life.

Key Responsibilities:
  • Prepare reports and case histories to document client progress and needs
  • Develop and implement individualized service plans to meet client goals and objectives
  • Provide companionship and support to clients, assisting with daily living activities and promoting social interaction
  • Assist clients with personal care, including bathing, dressing, and grooming
  • Prepare meals and snacks, ensuring clients receive nutritious and balanced diets
  • Conduct home safety assessments and provide recommendations for modifications to ensure client safety
  • Collaborate with healthcare professionals and other service providers to ensure comprehensive care
  • Participate in ongoing training and professional development to enhance skills and knowledge
Requirements:
  • Secondary (high) school graduation certificate
  • 1 year to less than 2 years of experience in a related field
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Physical ability to perform tasks that require standing, walking, and lifting
