Job Summary

The Discharge Planner plays a vital role in ensuring seamless patient transitions from hospital to home. As a key member of the interdisciplinary team, you will analyze clinical practices, identify areas for improvement, and develop effective discharge plans to optimize patient outcomes.

Key Responsibilities
  • Provide patient-centered care, adhering to the College of Nurses of Ontario's three-factor framework
  • Collaborate with healthcare teams to ensure efficient utilization of hospital resources
  • Develop and implement discharge plans, eliminating roadblocks to timely discharge
  • Communicate effectively with patients, families, and healthcare teams
  • Participate in quality improvement initiatives, promoting best practices and safe care
Requirements
  • RN Certificate of Registration from the College of Nurses of Ontario
  • Baccalaureate Degree in Nursing Sciences or related field
  • Minimum of five years recent and relevant Surgical clinical experience
  • Excellent communication, problem-solving, and leadership skills
  • Ability to work independently and as part of an interprofessional team
