Job Summary

We are seeking a highly motivated and experienced Discharge Planner to join our dynamic team. In this role, you will play a vital part in ensuring seamless transitions for patients as they move from the hospital to their next care setting. You will work collaboratively with interdisciplinary teams to develop comprehensive discharge plans that meet individual patient needs and promote optimal health outcomes.

Primary Responsibilities

  • Provide patient-centered care within the scope of practice for Registered Nurses, adhering to the College of Nurses of Ontario's three-factor framework.
  • Collaborate effectively with patients, families, and healthcare professionals to facilitate smooth transitions and ensure continuity of care.
  • Assess patient needs, develop individualized discharge plans, and coordinate resources to support successful transitions home or to other care settings.
  • Advocate for patients' rights and preferences throughout the discharge process.
  • Contribute to a positive and collaborative work environment by fostering teamwork and open communication.

Skills & Abilities

  • Registered Nurse Certificate of Registration from the College of Nurses of Ontario.
  • Baccalaureate Degree in Nursing Sciences or a related field.
  • Proven experience in discharge planning and case management.
  • Strong clinical assessment and problem-solving skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Commitment to patient safety and quality care.
