Job Summary

The Discharge Planner plays a vital role in ensuring the smooth transition of patients from the hospital to their next level of care. As a key member of the interdisciplinary team, this individual will be responsible for analyzing and reviewing clinical practices within the Surgical Continuum to identify opportunities for improvement.

Key Responsibilities

  • Develop and implement discharge plans that meet the unique needs of each patient
  • Collaborate with the healthcare team to ensure effective communication and coordination of patient care
  • Conduct daily reviews of patient status and revise discharge plans as necessary
  • Identify and address potential roadblocks to discharge to ensure timely and efficient patient flow
  • Provide education and support to patients and families to promote self-care and independence
  • Participate in quality improvement initiatives to enhance patient care and outcomes
  • Develop and maintain relationships with community resources and healthcare providers to ensure seamless transitions of care

Requirements

  • Registered Nurse (RN) Certificate of Registration from the College of Nurses of Ontario
  • Baccalaureate Degree in Nursing Sciences or related field
  • Minimum of five (5) years recent and relevant Surgical clinical experience
  • Collaborative working relationship with an interdisciplinary healthcare team
  • Effective communication and interpersonal skills
  • Ability to work in a fast-paced environment with multiple priorities
  • Strong problem-solving and critical thinking skills
  • Commitment to providing exceptional patient care and service
