Job Summary
The Discharge Planner plays a vital role in ensuring the smooth transition of patients from the hospital to their next level of care. As a key member of the interdisciplinary team, this individual will be responsible for analyzing and reviewing clinical practices within the Surgical Continuum to identify opportunities for improvement.
Key Responsibilities
- Develop and implement discharge plans that meet the unique needs of each patient
- Collaborate with the healthcare team to ensure effective communication and coordination of patient care
- Conduct daily reviews of patient status and revise discharge plans as necessary
- Identify and address potential roadblocks to discharge to ensure timely and efficient patient flow
- Provide education and support to patients and families to promote self-care and independence
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Develop and maintain relationships with community resources and healthcare providers to ensure seamless transitions of care
Requirements
- Registered Nurse (RN) Certificate of Registration from the College of Nurses of Ontario
- Baccalaureate Degree in Nursing Sciences or related field
- Minimum of five (5) years recent and relevant Surgical clinical experience
- Collaborative working relationship with an interdisciplinary healthcare team
- Effective communication and interpersonal skills
- Ability to work in a fast-paced environment with multiple priorities
- Strong problem-solving and critical thinking skills
- Commitment to providing exceptional patient care and service
