As a Mental Health & Addiction Therapist within our Integrated Mood & Anxiety and Addictions Concurrent Program, you will play a key role in a cooperative, interdisciplinary team aimed at providing cohesive services and programs to our clients. Your responsibilities will encompass treatment planning, clinical case coordination, and direct service delivery through both individual and group therapy for individuals facing complex mental health and addiction challenges.

Key Responsibilities:
  • Conduct and coordinate assessments to inform treatment planning.
  • Deliver case management services, including service planning, coordination, navigation, and progress evaluation.
  • Facilitate individual and group psychotherapy using evidence-informed methodologies such as Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed Care, Brief Solution-Focused Therapy, CBT, and DBT informed strategies.
  • Provide support to family members and lead group counseling and educational sessions.
  • Collaborate closely with Physicians and the clinical team to monitor patient experiences through regular meetings to assess progress, refine treatment plans, and communicate with referral sources.
  • Oversee the development and implementation of discharge planning and referrals to suitable community resources.
  • Identify discharge needs based on comprehensive patient assessments and care plans.
  • Lead service and team initiatives by establishing treatment objectives and collaborating with patients and team members to formulate actionable steps to achieve these goals.
  • Facilitate team monitoring and evaluation of treatment plan progress.
  • Act as a communication liaison among patients, the team, and the program, ensuring critical information is shared effectively.
  • Maintain documentation and record-keeping in accordance with organizational and regulatory policies.
Qualifications:
  • Bachelor's or master's degree in a recognized Social Science discipline with specialized education in addiction.
  • Membership in good standing with the relevant regulatory college/governing body in Ontario or actively pursuing relevant credentials.
  • Proficiency in delivering CBT, DBT, and other evidence-based treatments for complex mental health issues and substance use disorders.
  • Demonstrated experience in assessment and treatment utilizing evidence-based practices.
  • Exceptional communication and interpersonal abilities.
  • Understanding of the importance of measurement-based care in guiding clinical decisions; experience is an asset.
  • Ability to lead and facilitate an interdisciplinary team, optimizing team resources.
  • Skills in individual and group counseling, including the capacity to present psycho-educational content to larger audiences.
  • Capability to implement inclusive and culturally responsive approaches.
  • Knowledge of addiction issues and concurrent care across the treatment continuum.
  • Valid Ontario driver's license and access to a vehicle when necessary.
  • Familiarity with the Mental Health Act, PHIPA, Ontarians with Disabilities Act 2005, and Health Care Consent Act.
