VHA Home HealthCare Occupational Therapist Job Description

As an Occupational Therapist with VHA, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Key Responsibilities:
  • Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards.
  • Develop and implement client-centered care plans in collaboration with the client's integrated care team.
  • Ensure coordinated delivery of care through ongoing communication with all members of the client's team.
  • Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required.
  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Requirements:
  • Master's degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Valid Driver's License with access to a vehicle.
  • Compassionate and empathetic individual with excellent communication skills.
  • Ability to work independently and as part of a team.
  • Skilled in caseload management and electronic documentation.
  • Able to provide virtual care and possess a registered ADP authorizer (asset).
