Job Summary

The Indigenous Women's Healing Centre is seeking a skilled and compassionate Healing Plan Coordinator to join our team. As a key member of our staff, you will play a vital role in providing case management activities for residents/offenders, promoting their successful reintegration into the community.

Key Responsibilities
  • Develop and implement individualized case plans to address the unique needs and challenges of residents/offenders.
  • Conduct regular follow-ups and assessments to monitor progress and make adjustments as needed.
  • Collaborate with residents/offenders to identify and overcome barriers to successful reintegration, including substance abuse, mental health issues, and lack of education or employment.
  • Provide guidance and support to residents/offenders in developing life skills, such as budgeting, time management, and conflict resolution.
  • Work closely with community partners to access resources and services that support successful reintegration, including housing, employment, and education.
  • Develop and maintain accurate and comprehensive records of resident/offender progress and case management activities.
  • Participate in regular team meetings and contribute to the development of policies and procedures that support the mission and goals of the Indigenous Women's Healing Centre.
Requirements
  • Bachelor's degree in Social Services, Criminal Justice, or a related field.
  • Minimum 2 years of experience in case management or a related field.
  • Knowledge of Indigenous issues and culturally relevant approaches and practices.
  • Knowledge of risk assessment practices, risk management, and relapse prevention techniques.
  • Excellent organizational and time management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Ability to maintain confidentiality and handle sensitive information.
  • Ability to work in a fast-paced environment and adapt to changing priorities.
Preferred Qualifications
  • Knowledge of Indigenous languages and cultures.
  • Experience working with Indigenous communities and organizations.
  • Knowledge of community resources and services.
  • Ability to speak an Indigenous language.
