About the Role

We are seeking highly skilled Registered Nurses (RN) and Registered Practical Nurses (RPN) to join our team at Spectrum Health Care. As a key member of our MGH2Home Program in Toronto (East York), you will play a vital role in supporting client transitions from hospital-to-home.

Key Responsibilities

  • Provide high-quality nursing care to patients in the community, utilizing your clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Continuously assess and respond to the evolving needs of our patients, prioritizing their safety and well-being.
  • Travel within your designated region to provide care to patients in their homes.

Requirements

  • Minimum two years of nursing experience, with a preference for community nursing experience.
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community, with strong communication and interpersonal skills.
  • Proficiency in English, both verbal and written, with additional languages an asset.
  • Valid Ontario driver's license and reliable vehicle.
  • Experience with infusion therapy and wound care, with palliative care and mental health experience considered an asset.
