We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as part of our MGH2Home Program in Toronto (East York).

Job Summary

This is a contract position with guaranteed full-time hours working day shifts with weekend rotations as required. As an RN/RPN, you will support client transitions from hospital-to-home through our newly established MGH2Home program, working as part of a multidisciplinary team to support earlier discharges of adults with higher needs to reintegrate into their home and community.

Key Responsibilities
  • Provide high-quality nursing care to patients in the community, utilizing clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.
  • Work collaboratively with other members of the healthcare team to ensure patient-focused, safe, and holistic nursing care.
  • Continually assess, anticipate, identify, and respond to new and changing needs of patients.
  • Travel within your region between patients' homes.
Requirements
  • Minimum two years nursing experience, with community nursing experience being ideal.
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community.
  • Proficiency in English, both verbal and written required (additional languages an asset).
  • Valid Ontario driver's license and reliable vehicle.
  • Experience with infusion therapy and wound care.
  • Experience with palliative care and mental health considered an asset.
  • Excellent patient assessment and teaching skills.
