Position Overview

Nurse Practitioner (NP) - Medical Program
The Nurse Practitioner (NP) is a registered nurse in the extended class (EC) recognized by the College of Nurses of Ontario (CNO) and holds a Master's degree. NPs possess a legislated scope of practice that extends beyond that of a Registered Nurse in the general class, allowing them to initiate and perform controlled acts as per the Nursing Act and CNO standards.

The Nurse Practitioner exemplifies advanced expertise, skill, and judgment in delivering care to patients, adhering to the current CNO Practice Standards for Nurse Practitioners and the competencies outlined by the Canadian Nurses Association.

In this role, you will provide specialized and advanced nursing care to in-patients within the Medical program and Specialized Geriatric Services. You will work collaboratively with the interprofessional team and the most responsible provider to facilitate patient and family transitions throughout the healthcare continuum.

Your responsibilities will include conducting comprehensive nursing assessments and geriatric consultations for patients with complex healthcare needs. You will take on a leadership role, collaborating with the broader healthcare team and community partners to manage intricate patient care scenarios.

Additionally, you will partner with program leadership and frontline staff to develop, implement, and evaluate best practices in geriatric care across the organization. You will engage in professional and organizational initiatives to advance nursing practice and enhance patient care outcomes.

Key Responsibilities


• Provide care in inpatient, outpatient, and outreach settings, offering rotational NP coverage across various care environments.

• Conduct thorough nursing/medical histories, physical, and psychosocial assessments of patients, and develop care plans to address their needs.

• Deliver specialized care and/or consultations to patients, families, healthcare professionals, and community partners in complex situations requiring advanced nursing expertise.

• Collaborate with the interdisciplinary team to establish care goals and advance end-of-life care planning.

• Lead initiatives to establish and meet nursing and clinical practice goals, ensuring alignment with the strategic direction of the program and organization.

• Initiate and perform diagnostic and therapeutic procedures as authorized through medical directives when necessary.

• Contribute to the development and advancement of best practice standards in senior care in alignment with the organization's strategic direction.

• Collaborate with the interdisciplinary team and external partners to assess and monitor patients with cognitive impairments and develop individualized care plans.

• Work with community providers to ensure home-based services meet the needs of individuals and families.

• Participate in continuous quality improvement initiatives within the serviced organizations.

• Develop, implement, and evaluate clinical service programs in collaboration with the Program Director and Director of Professional Practice.

• Engage in nursing research and participate in interprofessional research projects as required.

• Involve in scholarly activities, including presentations and publications.

Qualifications


• Registration with the College of Nurses of Ontario (CNO) in the RN (Extended Class) and a member in good standing is required.

• Completion of an Acute Care Nurse Practitioner Program (or equivalent) is required.

• CNA certification in Medical-Surgical Nursing and/or Gerontology is an asset.

• Certification in Hospital and Acute Care for Nurse Practitioners (or equivalent) is an asset.

• Completion of specialized courses such as PIECES, GPA, or Montessori is an asset.

• Membership in relevant professional organizations (e.g., NPAO, RNAO) is an asset.

Required Experience:

• A minimum of 3 - 5 years of clinical nursing experience in acute care medicine, geriatrics, geriatric psychiatry, and/or chronic disease management is required.

• Previous experience as a Clinical Nurse Specialist or Nurse Practitioner in acute care is preferred.

• Eligibility for cross-appointment with the Faculty of Nursing, University of Toronto, or another academic institution is desirable.
